In addressing the national objectives set forth by HUD (see our previous post), HSN has ongoing strategic goals which frame the action plans created for each year’s Continuum of Care application. These goals may change as new research and data about our Continuum of Care dictate, and are reviewed annually.
ONE: Implement rapid re-housing of homeless persons within their communities by offering sufficient housing capacity to serve those in need
A — Expand availability of rent/utility assistance for precariously housed, including those facing eviction
B — Expand funding for motel/hotel vouchers for homeless individuals and families when shelters are at full capacity
TWO: Lead the development and availability of affordable rental housing for the homeless and low income residents of Central Florida
THREE: Remove barriers to community reintegration of homeless individuals and families
A — Establish a Homeless Court to dispose of criminal charges, court fees and fines
B — Develop ongoing monthly access to one-stop ID center to obtain birth certificates and identification needed for housing, employment and access to services
C — Implement regional Community Voice Mail system for homeless and low-income families
FOUR: Expand the reach of the Health Care Center for the Homeless to provide a health care system for the homeless in all three counties
FIVE: Expand mental health and substance abuse services to the homeless to improve access to detox, crisis stabilization, and treatment facilities.
SIX: Develop and implement a comprehensive housing discharge planning process for the region’s correctional facilities and hospitals to ensure that clients are not discharged to homelessness.
A — Increase transitional housing for recent homeless discharges from hospitals and mental health/substance abuse facilities.
SEVEN: Implement a regional “No-Wrong Door” homeless services referral process
A — Develop team of assessment specialists to provide effective housing triage in a centralized process to swiftly serve client needs and direct clients to available shelter
B — Develop and implement a universal intake form to consistently assess barriers to rehousing
C — Expand the use of ServicePoint as the Continuum-wide HMIS to coordinate the referral and service delivery process for clients in the Continuum of Care.
D — Develop a network of homeless prevention centers to provide convenient access to assessment, referral and direct delivery of urgently needed services for the homeless.
E — Develop a comprehensive regional outreach program to unsheltered homeless individuals and families to expand access to basic services and housing
