CoC Program Performance Report Card

Homeless Services Network will roll out a new program performance report card as part of its annual review process for Supportive Housing Program project renewal process. This report will inform the HSN Board review process for the annual Continuum of Care application for funding.

The report card includes the following data points:

  • Bed utilization rate (average bed occupancy for the last full year of grant operations)
  • Cumulative number of clients served, compared to grant goal
  • Cost per client served
  • Funds (if any) returned to HUD through non-expenditure by sub-recipient

Program performance outcomes:

Outreach/supportive services only programs:

  • Cumulative number of clients outreached to, compared to grant goal
  • Number of clients moved into treatment, and/or housing
  • Number of clients who obtained jobs (employment grants specifically)

Transitional housing programs:

  • Number and percent of households served who moved into permanent housing in the community

Permanent supportive housing programs:

  • Number and percent of households who maintained housing stability for at least six months

ALL housing programs:

  • Number and percent of clients who increased their income through employment and/or mainstream benefits
  • Number and percent of clients who had SSI/SSDI at discharge from program
